Prior to your Appointment/Day of Appointment

After your appointment is scheduled, you will receive an email to access the portal. Please read and complete all documents in the portal prior to your appointment. Your time is important to us. The following information is needed on the day of your appointment to ensure that your appointment process goes smoothly and efficiently:

• Driver’s License or valid photo ID to verify identity
• Medical Insurance Card(s) (If no insurance card is provided at the time of our appointment, you may be asked to self pay or reschedule your appointment)
• Completed Registration Paperwork (Preferred completion via Portal prior to appointment; if unable to complete via portal prior to appointment, then please arrive 15 minutes prior to appointment time to allow electronic check-in and completion of forms in the clinic.)
• Copay/Deductible/Coinsurance Payments are due at the time of service
• Faxed Referral from primary care physician or referring physician if it is required by your insurance.

Please arrive 15 minutes early to your appointment to allow time for staff to complete the administrative portion of your appointment. You may be asked to reschedule if you arrive more than 15 minutes after your appointment time or do not have the above listed items. If you anticipate running late to your appointment, please call and notify our office and we can verify if you can still be seen or may need to be rescheduled. There is a $50 fee for no shows or late cancellations less than 24 hours from your scheduled appointment.
